Ordering and Submission Process
How To Start An Order
Starting an order with Quorum begins with a case review and submission of the information needed to design and fabricate a custom 3D printed prosthetic solution.

1. Contact Quorum
Reach out to Quorum to discuss the case, requested device, patient needs, timeline, and any questions before submitting materials.

2. Gather Case Information
Collect the patient identifier, amputation level, side, mobility level, activity goals, fit concerns, skin considerations, and any relevant clinical notes. Use the Socket Order Form to organize the required case details.

3. Capture or Prepare the Scan
Submit the scan references needed for the case type. For a definitive 3D printed socket, include a scan of the well-fitting check socket positive model, or filled cast, and a scan of the fit and aligned check socket with the complete foot or distal pyramid for alignment transfer.

4. Submit Device and Alignment Details
Provide the requested device type, liner info, suspension method, componentry, alignment notes, build height concerns, and any special instructions. For definitive sockets, include the foot, knee, adapter, pyramid, or component details needed to help transfer alignment accurately.

5. Send The Completed Order Form
Email the completed Socket Order Form, scan references, photos, measurements, and supporting case details to sales@opquorum.com for review.

6. Quorum Reviews the Case
Our team reviews the submitted information and may contact your clinic with questions, recommendations, or design confirmations before fabrication begins.

7. Design and Fabrication
Once the case details are approved, Quorum designs and fabricates the custom 3D printed prosthetic solution based on the submitted clinical information.

8. Shipping, Fitting, and Follow Up
The completed device is shipped to your clinic for fitting. Your practitioner team completes the patient fitting, reviews comfort and alignment, and gathers feedback. If adjustments or additional support are needed, send fit notes, photos, and feedback to Quorum so our team can help determine the next steps.
